In some cases, you might not feel a strict lump in your testicles, but changes in size of firmness in your testis. This is more common in testicular cancer than benign growths because when there’s a lump within the testicle itself, it is more likely to be testicular cancer. This type of internal lump would not feel like a bulge or protrusion but may instead give you a change in how the testicle feels as a whole.

One testicle is always bigger than the other, but the difference is difficult to note and often only measurable by ultrasound. When there’s a big difference in size, it is advised to ask your doctor, and he will perform a complete physical exam. On the other hand, you may also feel a difference in texture and firmness. Testicular cancer increases the density of the testicular tissue and may feel tense, often harder than the opposite testicle. Of note, these changes in the structure of the testes are not often accompanied by pain. So, do not rule out testicular cancer just because it is painless, and ask your doctor when in doubt.
