Another commonly occurring symptom of protein deficiency is a fatty liver which indicates the severity of protein deficiency. In case of fatty liver, there is accumulation of fat around the liver cells. If a person does not take mandatory steps to treat protein deficiency, the condition might turn into fatty liver disease. Fatty liver disease can lead to liver scarring, inflammation, and most probably liver failure.

Fatty liver occurs more commonly in people who are avid drinkers of alcohol as well as obese people. The exact pathophysiology of fatty liver due to protein deficiency is unclear but scientists suggest that the impairment in synthesis of lipoproteins, the fat-transporting proteins might lead to the disorder.
