Abdominal swelling ( abdomen distension ) is a common finding in hemolytic uremic syndrome (HUS). This is caused by acute inflammatory spasm of the smooth muscles of the intestines, which results in increased peristalsis, slow transit of intestinal contents, and backup with fluid and gas. This leads to pain, nausea, vomiting and diarrhea with formation of straining diarrhea containing mucus and blood.

Abdominal swelling with hemolytic uremic syndrome is often referred to as being bloated, blocked or full, swollen belly, and water belly. Any of these symptoms can be encountered in the first day or weeks of HUS because of the blockage caused by swelling.
